Medical Weight & Wellness Consultation Required Before Enrollment in Any Wellness Injection Program This comprehensive consultation with our Nurse Practitioner is required prior to starting any provider-directed wellness injection program. During this visit, we will review your medical history, current medications, lifestyle habits, nutrition, activity level, and wellness goals to determine the safest and most appropriate treatment plan for you. Includes: • Comprehensive health history review • Medication and risk assessment • Nutrition and protein evaluation • Goal setting and wellness planning • Discussion of available treatment options • Personalized recommendations from the Nurse Practitioner Completion of a consultation does not guarantee eligibility for treatment. All recommendations and treatment decisions are based on the Nurse Practitioner’s clinical assessment.

Wellness Injection Follow-Up For Established Wellness Clients Only This appointment is reserved for clients who have completed their initial Nurse Practitioner consultation and are actively participating in a provider-directed wellness program. Includes a brief progress review and administration of your prescribed wellness injection, when appropriate. Initial consultation with the Nurse Practitioner is required before scheduling this service.

The treatment works by injecting small amounts of Botox in the upper lip muscle, which causes a slight relaxation of the muscle and allows the lip to “flip” outward, giving the upper lip, a fuller, and more defined look last 3 to 4 months
